Event Details

AHEC West Diabetes & Prevention Program, for ages 18 and and older. The Diabetes Prevention Program includes lessons, handouts, and other resources to help you make healthy changes. A lifestyle coach, specially trained to lead the program, to help you learn new skills, encourage you to set and meet goals, and keep you motivated. You will learn to eat better without having to give up the foods you love and add physical activity such as taking a walk to your weekly schedule. The coach will also facilitate discussions and help make the program fun and engaging. A support group of people with similar goals and challenges. Together, you can share ideas, celebrate successes, and work to overcome obstacles. In some programs, the participants stay in touch with each other during the week. It may be easier to make changes when you’re working as a group than doing it on your own.

To be eligible for the Diabetes Prevention Program, there is some eligibility criteria. Someone could be either diagnosed with prediabetes from their provider or they can take a “self-assessment” quiz. The quiz is really easy. For your online registration form, could you reference the online quiz from the CDCs website? The language to reference it can say: “Find out if you are at risk for prediabetes and type 2 diabetes.
